Information Collection, Use, and Sharing

“Personal information” means information capable of identifying, directly or indirectly, a specific person or household. We access and collect personal information that you voluntarily give us, such as your name, address, telephone number or email address, through registrations, applications, surveys, support or information requests, or other data capturing processes. Healthcare professionals may choose to provide us with information relating to their specialties, and professional status and affiliations. We may combine personal information we have about you with information obtained from public sources.

We will use your personal information to respond to you, regarding the reason you contacted us. Unless you request us not to, we may contact you via email in the future to tell you about specials, new products or services, or changes to this Privacy Policy.

This Privacy Policy does not cover any information collected during clinical trials or other research endeavors that CAMP4 participates in, sponsors, or is affiliated with. For information about clinical trials, refer to the Information and Consent form for such trial or study. Please note that if you provide personal information to CAMP4 through a sign-up form and indicate that you wish to learn more about a clinical trial, we intend to share this information with clinical trial sites conducting trials so that they may contact you about the possibility of your enrolling in one or more trials.

This Privacy Policy also does not apply to personal information relating to employees, job applicants, and contractors, who will be provided a separate Privacy Statement from CAMP4 where required by law.

If you provide personal information to us, you acknowledge and agree that such personal information may be transferred from your current location to CAMP4’s offices and servers and/or those of the authorized third parties referred to in this Privacy Policy, located in the United States.
